Asset Update Request

The following instructions describe how to complete the Asset Update Request form.
To log in to the form, go to
Enter your NVCC Username (your local userID, not your NVCC email address) and Password.
After successful login, the Forms Library dashboard opens. Click on eForms in the left panel or under Forms Library to view the forms.
Click on 105-137 Asset Update Request.
The form opens. Your Requestor Email and Name fields should be already populated.
Complete the Current Asset Information, and upload Manifest documentation (optional).
In the Manifest section, attach files, as applicable, and click Upload Manifest.
Attach supporting documentation, and click Submit.
